Have your screenshot in a place/folder you will remember, I recommend your desktop.
Go to the reporting form (https://mcpz.net/applicationform/user-report.4/form) and press the 'Upload a File' button.
Find the image you saved, click on it and use that. It will automatically turn into an image we can see once you submit the report.
Job done!
