It's like making a sign, but Instead of the 6 wood it's wool. After making a banner (use any 6 color wool of the same color to get that color banner) then use dyes (any colors) and start placing them with the banner.(in the crafting table) You can also use a creeper, wither skull to have their faces on it or use a notch apple to put the Mojang symbols on it. If you have a color with the banner and the heads/apple in the crafting table, you would change the faces/apple symbols to the color chosen. With the color patters you might need to try different crafting recipes or google them.
