Is there any specific format that people need to use in a helper application, or any application for that matter?

I do not plan to apply for Helper just yet; this is for future reference.

If you go to the place to apply for helper there's already a format that you fill in, you don't need to write it, just answer the questions.

Yeah. Just hit "APPLICATIONS" and then "Apply for Helper" and you can.
