What is the usual proof to confirm someone uses xray? For example: looks at walls or walked to this player with walls between them?

It depends. Seeing as X-ray is a hard modification to verify we tend to obtain as much evidence as possible. This can be:

• Continually following a user through a wall or staring at them.
• Instantly finding the location of another player.

However people can be using F5 if it's just round a corner. So it's hard to tell, the more evidence the better.
