do you use shaders?, if u do maybe try not using it

^
In addition, you might be running out of memory?
If you are running out of memory, you just usually like lag alot and stuff but it could still be a possibility.

Wait you're using minecraft version 1.9.4, try using 1.8.9, And also you apparently ran out of memory, try removing the texture packs or shaders if you were using any of them
