i have a screenshot of a player hacking but i don't know how to upload it to the form. could you please explain how to do it?

A single screenshot of a player hacking is 99% of the time insufficient evidence to prove hacking. There are exceptions to this, of course, but a video is preferred. You can upload images on the report form (https://mcpz.net/applicationform/user-report.4/form) with the 'Upload a File' button!
